SMPH Grant Library Consent

The School of Medicine and Public Health (SMPH) has developed a Grants Library with a Grants Library Repository (GLR) to serve as a centralized resource for grant writers/investigators at different career stages to allow access, through an application and approval process, to awarded training, research, and career grant applications as well as other general stock language and proposal templates and tools.

As a successful grant recipient, we kindly request your permission to share full copies of either successfully completed (closed) proposals (Section A: Option 1), and/or copies of specific sections of successful proposals (closed or open) (Section B: Option 2). Note that you must choose Section A, Section B, or both Sections A and B with this consent form to access the necessary fields. After selecting an option(s), you will be able to upload a file and provide further directions to Grants Library Repository staff members.

Please refer to Sections A and B below for option details. Any type of federal or non-federal grant is welcome and accepted. You may use a single form to submit multiple proposals (and/or sections of proposals).

Section A: Option 1

Option 1 – Full Grant Application: As a successful grant recipient, we kindly request your permission to include an edited, read-only version of your completed (closed) grant in the Grants Library Repository. This will help support our office in its mission to advance the SMPH research enterprise and assist current trainees, scholars, new investigators, colleagues, and future collaborators to strengthen and improve their own applications. Measures will be taken to ensure ethical use of read-only content. Access to shared grant proposals will be available only to SMPH faculty (or faculty on behalf of their research trainees), scientists and applicable staff with an active NetID account upon completion of a vetted application and approval process. *If you consent to share multiple grants, you may use a single consent form listing all grants in the form.

Section B: Option 2

Option 2 – Application Sections: We request your permission to include an edited, read-only version of specific section(s) of a successful proposal (current or closed) in the Grants Library Repository. Measures will be taken to ensure ethical use of read-only content. Example stock language documents listed in columns labeled Attachments and Compliance Sections below will be available to anyone with access to the Grants Library website, while access to the documents listed in Research Plan Sections will be limited to SMPH faculty/staff with a valid NetID account. *If you consent to share sections from multiple grants, you may use a single consent form.
